Green on ID/epilepsy/mitochondrial/metabolic panels. Well-established AR mitochondrial disorder typically presenting in infancy. PMID:32088416 - prenatal onset of profound microcephaly with intrauterine growth restriction (IUGR). PMID 36049610: patient 1 with TRIT1 R327X/G251R - IUGR and microcephaly on 2nd trimester ultrasound, MRI at 3 months showed vermis hypoplasia, thin corpus callosum, enlarged ventricles, polymicrogyria. Abnormalities of corpus callosum noted postnatally in at least 4 cases, structural cardiac defects (mostly septal defects) also noted postnatally in at least 4 cases (PMID: 36047296).

Clinical review and curation was performed by Lyn Chitty, (North Thames GLH), and Stephanie Allen, Denise Williams, Anna de Burca and Megan Horton-Bell (Central & South GLH). Outcome of review: Confirmed that the phenotype is fetally relevant, support adding to the Fetal anomalies panel as a Amber gene.

On 6 panels, inc. IEM, severe paediatric disorders and DDG2P. Associated with Combined oxidative phosphorylation deficiency 35 (AR). Rare condition, most cases present around 3-6 months of age. PMID: 32948376, PMID: 36049610 2 more cases with antenatal onset? If 2 extra cases showed IUGR +/- microcephaly, consider including. Forde et al., 2020 PMID 32088416: Px: IUGR from 28wks gestation. Born at 35+2wks, birth weight 2nd-9th centile and OFC <0.4 centile. Neonatal: dysmorphic features, moderate atrial septal defect, bilateral polymicrogyria and thin CC. Conclusion: weak link to prenatal phenotype (IUGR). ?Relevant
